In order to ensure the appointments you will be attending will be appropriate and of use to you I will collect the following forms of personal information.
This will be provided to me directly by you in an initial telephone consultation and during your first appointment which will be at your request.Name and Date of birth

How I process and share personal information
I have regular supervision/consultation with a supervisor who will be a therapist or Psychologist in order to ensure good practice in delivering psychological therapy and for my own professional development.I may discuss some personal details in these supervisory consultations, however I will never identify an individual by using a full name. My supervisors are bound by the same professional rules of confidentiality, codes of ethics and rules of GDPR.Under the UK General Data Protection Regulation (UK GDPR), the lawful basis I rely on for processing this information is your consent. You are able to remove your consent at any time. You can do this by contacting me at [email protected]
In the event of
an emergency
If I have reason to believe that you intend to harm yourself or another person/organisation (eg. Terrorism), the law may require that I inform an authority without seeking your permission.In such a situation, the law may require that I share your personal information without your knowledge. I would always endeavour to discuss this with you in advance if possible or appropriate.It is also the law that a judge can require the release of clinical notes without your permission, and I would be bound by law to release them. Again, I would discuss this with you beforehand should this occur.
Storage of
information
Paper: Contact details, personal information, psychometric tests if/when completed, assessment/history taking notes, occasional email correspondence, occasional therapy notes, and invoices. All paper files/notes are stored in a locked filing cabinet in an office.Digital: I may keep your name and contact details in one smart phone and laptop (whilst you are attending regular appointments). Backups are made to an external hard drive. All devices and backups are password protected.Erasing your Information: I keep electronic emails and SMS/texts for up to a year after regular sessions have ended. I am required by my professional insurance company to keep any clinical notes for up to 7 years after we have completed our work together. After this time has passed, I will shred and delete the information.
